This form is required for all athletes competing with us for the first time or any athlete who has not competed for three years or longer. It can be completed by the athlete, a family member, or a provider.
This form is required for all athletes competing with us for the first time or every three years for a returning athlete. The form must be completed and signed by a medical professional to be accepted.
This form is required yearly for all returning athletes. It can be completed by the athlete, a family member, or a provider.
Documents should be scanned as a PDF and emailed to delawarecountyracers@sooh.org

Registration is not complete until the Local Coordinator reviews the documents.
This is an OPTIONAL form.
You do not have to have a physical at Minute Clinic, your normal doctor can fill out the medical form above.
If you would like to take advantage of Special Olympics' deal with the Minute Clinic for a physical, fill out this form and take it to your appointment to get the discount along with the medical form above.
Delaware County does not need a copy of this form.
All Special Olympics Ohio athletes are eligible to obtain a renewable year-long membership at their local YMCA branch at no charge. Memberships should be renewed yearly using the same form below.
Eligibility
An athlete must have current physical on file and be in good standing with their local organization.
Athlete’s are eligible to obtain membership for a private provider if necessary.
Private providers are only allowed to use their free YMCA membership when accompanying an athlete.
The first step is for the athlete to complete the YMCA Membership Request Form.
Athlete enters their full name and contact information (email, phone). If the athlete does not use email, they should enter the email of someone who can receive their membership certificate.
Choose the athlete’s Local Organization from the menu (by clicking the +Add button). We are "Delaware Special Olympics."
If the athlete requires a private provider with them, answer that question "yes" and a place will appear for the provider's name. If the provider changes from day to day, enter one main provider’s name.
You need to choose a YMCA location. Again, click the +Add button to see a list. Likely choices for our athletes are "Delaware YMCA (Delaware OH)" and "Liberty Township/Powell YMCA (Powell OH)".
The athlete and our Local Coordinator will receive an email confirmation. The Local Coordinator will approve the membership as long as all eligibility requirements are met.
